Passports for Children

Click on http://www.travel.state.gov/passport/get/minors/minors_834.html for complete information about passport issuances to minors.  Applicants under 16 years old must apply in-person with both parents and submit the following:

• Completed form DS-11     http://travel.state.gov/passport/forms/ds11/ds11_842.html
• Proof of U.S. citizenship
• Evidence of child's relationship to parents or guardians
• Parents' photo identification
• Statement of Consent or Special Circumstances, if applicable (see below) 
• Two identical passport photos taken within the last 6 months (2" by 2" on white background)
• Fee of $85 USD

We strongly encourage all applicants to complete the passport application on-line.  The on-line application form assures efficient and accurate processing of your application.

NOTE: If only one parent is in the country, the absent parent's consent to the issuance of the child's passport must be proven by a signed and notarized Statement of Consent or Special Circumstances (Form DS-3053), available on-line at:

NOTE: Minors between the ages of 16-18 should be accompanied by at least one parent.
