Passports for Children
Click on http://www.travel.state.gov/passport/get/minors/minors_834.html for complete information about passport issuances to minors. Applicants under 16 years old must apply in-person with both parents and submit the following:
Proof of U.S. citizenship
Evidence of child's relationship to parents or guardians
Parents' photo identification
Statement of Consent or Special Circumstances, if applicable (see below)
Two identical passport photos taken within the last 6 months (2" by 2" on white background)
Fee of $85 USD

NOTE: If only one parent is in the country, the absent parent's consent to the issuance of the child's passport must be proven by a signed and notarized Statement of Consent or Special Circumstances (Form DS-3053), available on-line at:

NOTE: Minors between the ages of 16-18 should be accompanied by at least one parent.
