Passports

Passports for Children

Click on http://www.travel.state.gov/passport/get/minors/minors_834.html for complete information about passport issuances to minors.  In general, applicants under 14 years old must apply in-person with both of their parents and submit the following:

  • Completed form DS-11 (available at the Kingston, Montego Bay and Grand Cayman consular locations or through http://www.travel.state.gov/passport/get/minors/minors_834.html
  • Proof of U.S. Citizenship
  • Evidence of Child's Relationship to Parents or Guardians
  • Parental Identification
  • Parental Application Permission Documentation
  • Two identical passport photos taken within the last 6 months (2" by 2" on white background)
  • Fee of $70 USD

Renewals of Children's Passports: Renewals for children can be applied for in-person or through the mail. Please click on Renewals by Mail link for more information.  Applicants will need the following:

  • Your previous passport (photocopies NOT accepted)
  • Completed application form; DS-82 if renewing by mail (forms available at each consular location or at http://www.travel.state.gov/passport/forms/forms_847.html)
  • Two identical photographs taken within the last 6 months (2" by 2" on a white background)
  • $55 USD renewal fee

NOTE: Renewals usually take several weeks, so please renew the passport in advance of its expiration date, if possible, to avoid travel delays.
